Insurance Carrier · Underwriting and risk review

Review the documentation behind the treatment workflow.

For interventional mental health practices, Arcametric can help preserve reviewable preparation, administration, safety and follow-up records. The underwriting question is whether an insured’s actual use produces meaningful evidence for a carrier's risk-control process.

Four evidence questions

Underwriting concernReviewable informationCarrier should verify
Preparation before careRecorded preparation, baseline context and supported consent/safety facts.Which local forms, practitioner attestations and screening records actually exist?
Events during extended sessionsTreatment start and end, documented doses, vital signs, observations and intervening events. Are timestamps, event sequence and omitted intervals clear for the insured case?
Safety responseRecorded safety events and linked action, severity and outcome where available.Which contemporaneous emergency, incident, policy and clinical records corroborate the event?
Transition and follow-upPlanned work, due dates, completion, integration and assessment history.Did the insured complete applicable responsibilities, and what evidence establishes that?

Two separate review lanes

Clinical documentation and malpractice

Record consistency, chronology, missingness, event response and follow-up evidence.

Data protection and cyber

Identity boundary, access permissions, exported data, contracts, retention, incident handling and controls requiring independent verification.

No actuarial or legal conclusion. Structured documentation does not itself establish that care met professional standards, reduce claim frequency, establish coverage or justify a premium credit. A case-specific review and carrier methodology remain necessary.
Insurance Carrier · Representative evidence

What a reviewable record could contain.

A reviewer should be able to separate actual source facts from gaps, summaries, and records maintained outside Arcametric.

Review itemStructured entryWhat remains to be checked
PreparationRelevant preparation completed status and documented baseline facts.Signed consent forms, required site policy and practitioner responsibility.
Administration10:05 · 50 mg ketamine dose event, with linked treatment occurrence.Source attribution, local chart, indication and professional authorization.
Monitoring10:20 · blood pressure 128/82 as separately timed measurement. What was monitored in unrecorded intervals and any external device record.
SafetyAn event is included only if an eligible supported record exists.Escalation, EMS/hospital information or other external incident materials.
Follow-upScheduled items and documented completion/overdue state.Any outreach or services outside the Arcametric workflow.

How to evaluate an insured practice

Request a bounded sample

Obtain an authorized coded-case chronology and underlying clinical evidence appropriate to the review.

Check reliability

Inspect source, timestamps, recorded omissions and differences between source records and rendered output.

Apply carrier criteria

Determine relevance within underwriting, risk management or claim-review practice using carrier-defined standards.

Related operational benefit

A structured chronology may help both practitioners reviewing continuity and risk managers assessing the documented workflow. That is complementary value, not automatic sharing of the same person-level records with a carrier.

Privacy boundary. Direct identity is held by the identity-owning practice or organization; coded references and limited identifiers do not automatically meet a statutory de-identification standard. Relevant privacy, contract and transfer questions require separate review.
Insurance Carrier · Optional diligence annex

Ten questions for a technical review.

This questionnaire is a discussion aid, not an underwriting score, mandated control set, or attestation of an individual insured’s actual conduct.

DomainEvidence question
Preparation1. Which required pre-session checks and consent steps can the insured demonstrate for sampled encounters?
Medication review2. What documents the practitioner’s medication and interaction review, distinct from reference lookup?
Chronology3. Can an authorized reviewer reconstruct recorded start, dose, monitoring and end events without inferring omitted actions?
Vital signs4. How are values, measurement times and gaps distinguished?
Safety5. How are adverse events, reported reactions, responses and outside incident records connected?
Follow-up6. Which plans and due items were documented, completed, missed or left unresolved?
Outcomes7. Which standardized assessments were actually collected at comparable timepoints?
Record integrity8. How can reviewer validate source, relevant amendments and the scope of each report or export?
Identity boundary9. Where is the name-to-code mapping controlled and what access or disclosures are permitted?
Cyber controls10. What deployment-specific evidence supports access security, retention, vendor agreements and incident response?

Preferred next action: a bounded evidence review

For an introduction from a broker, insured organization, risk adviser, or counsel, request a sample record and identify which documented elements could inform the carrier's existing review process. No rating or underwriting change is presumed.
